Job Description\n

American Traveler is seeking an experienced RN Care Manager for a hospital-based leadership role requiring care management and utilization review experience.

\n \n Responsibilities\n

• Work is based in a hospital setting providing care management services
• Assignments are for 13 weeks with 40 hours per week
• Shifts are variable: either weekday 8:30 am to 4:30 pm, or four 10-hour shifts from 7:30 am to 5:00 pm
• No on-call required
• Weekend and holiday rotations are expected
• Role involves concurrent utilization management and discharge planning across all inpatient units
• Responsible for assessing patient needs and facilitating transitions to appropriate care settings
• Lead and collaborate with an interdisciplinary team during care rounds and conferences
• Coordinate with community organizations for durable medical equipment, insurance authorizations, and patient transportation
• Utilize strong written, verbal, and electronic communication skills, including proficiency with relevant hospital software
• Participate in quality improvement initiatives and apply knowledge of hospital reimbursement and quality measures
• Facility may refuse or terminate assignments based on internal policies

\n \n Requirements\n

• Active RN license required
• Valid BLS (Basic Life Support) certification required
• Minimum of 3 years of clinical experience in med/surg nursing, home care, hospital case management, or utilization review
• Prior experience with both utilization review and discharge planning is required
• Associate or Bachelor's degree in nursing required
• National case management certification (CCM or ACM) is preferred
