Job Description

MSA Home Health and Hospice , a proud member of the Medical Services of America family, is currently seeking an experienced Part-Time/PRN Home Health Social Worker for our Home Health location in Smyth Co, Washington Co. (Smyth Co, Washington Co) VA.

• Provides services in accordance with the physician’s plan of treatment and assists the physician and other members of the home health team to understand the significance of social and emotional factors related to the client’s care.
• Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families.
• Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies.
• Recognizes and utilizes community and family resources to assist the client in long-term plans for life in the community or to learn to live within the client’s disability.
• Cooperates with appropriate staff to provide staff education when requested and arranged by the client care manager..

Job Requirements

• Bachelors or Masters degree in social work, sociology, psychology, or rehabilitation counseling form an accredited 4 year college or university
• Minimum of one year experience in social work, preferably in a home care agency, hospital, rehabilitation center or mental health program
• Currently licensed to practice as a social worker in the state of VA.
• Valid Driver's License.
