Provides in home evaluation, direction and application of skilled Nursing Services to complement the patient’s overall Plan of Care. Case Management ensures the overall patient condition is cared for by providing appropriate education and resources to the patient/caregivers, allowing them to manager their conditions and remain at home. Job Requirements Education Required: Graduate of an accredited nursing program Experience Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years. Skills Ability to provide reliable transportation as required by nature of position. Licensure/Certification/Registration Required: RN License active in the state of Missouri Required: BLS must be obtained within 90 days Required: Active Driver's License
