Registered Nurse (RN) – Home Health – Santa Cruz County

Job Type: Full-Time

Schedule: Flexible – Set Your Own Hours

Compensation: Point-based pay based on years of experience with a range of $60-95

We’re looking for compassionate, motivated Registered Nurses to join our growing Home Health team in Santa Cruz County . In this field-based role, you'll provide high-quality, personalized care to patients in the comfort of their homes. You'll help clients recover, stay independent, and feel supported—while enjoying autonomy, flexibility, and meaningful patient relationships, mainly with seniors.

Compensation – Simple, Transparent & Rewarding

We use a point-based pay system where each visit type has a point value, and your point rate is what you earn per point. For example, if your point rate is $75, here’s how it breaks down:

Earnings Examples (at $60/point):

• Start of Care (2.5 pts) = $187.50
• Resumption of Care – Nursing (2 pts) = $150
• Recert, DC OASIS – Nursing (1.5 pts) = $112.50
• Routine Visit (1 pt) = $75

Full-Time = 30 points/week = $2,250/week

What You Need to Join Us

• Active RN license in California
• 1+ year of recent experience in home health or hospice
• Experience with OASIS and electronic charting
• Strong clinical assessment and organizational skills
• Ability to educate and support patients and families
• Reliable transportation to travel within your assigned territory
