Skilled Clinical Experts LLC is seeking a full-time Nurse Practitioner (np) to join our rapidly growing remote/in-home chronic care management practice. As an SCE field provider, you will treat and manage patients with multiple chronic disorders and/or transitioning from a hospital/skilled facility to the home setting. Care is provided both in-person, in the patient’s place of residence and via audio/video. Experience working remotely and independently is recommended with experience in geriatrics preferred.

* Reduce unnecessary and costly hospitalizations and/or rehospitalizations

* Most importantly, help our patients improve their quality of life while reducing their overall spend

 

Responsibilities:

• Diagnose and treat acute, episodic or chronic illnesses
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Initiate referrals to other healthcare professionals and medical facilities
• Communicate with a collaborating physician, patient care coordinators, or specialists regarding patient care

Qualifications:

• Current and active Illinois license
• DEA
• Experience providing independent direct patient care
• Ability to document within an HER
